【中文摘要】 农民工群体是中国改革开放后出现的一个特殊的群体,他们是指具有农村户口身份却在城镇务工的劳动者,但由于传统的城乡二元的经济体制和户籍制度,他们的身份并没有得到广泛的认同。农民工们往往享受不到和城市人一样的市民待遇,很多体制性障碍常常制约着他们的生存和发展。最需要医疗保障的农民工群体却被排除在社会医疗保障之外就是最典型的现象之一。农民工问题已经日益引起社会关注,号召社会关心和帮助农民工的呼吁也越来越多,国内研究者们也从不同角度对我国农民工问题进行了各种分析。本文通过横向对比分析,系统的运用社会公平理论、制度与秩序理论和冲突理论,使以上理论与社会医疗保障的具体问题相结合,并针对我省农民工医疗保障现有状况,进行认真的探索分析,通过分析发现了影响吉林省农民工医疗保障发展的障碍,最后,通过对吉林省相关政策法规、社会主观认识、农民工自身特点等内外因素调查分析,提出了解决吉林省农民工医疗保障问题的基本原则及具体对策:(1)逐步消除吉林省城乡户籍的影响(2)完善相关法规(3)促使吉林省农民工医疗保障基金的保值增值(4)做好吉林省农民工的统计工作和问题研究工作等若干政策及建议。在课题研究中我们以问卷方式对吉林省长春、通化等市的100位农民工、50位市民进行调查,并随机抽取了吉林省具有代表性的10家用人单位和10个社区进行了调研,内容涉及年龄、身体健康状况、学历层次、保障需求、用人情况、医疗保障状况等几个方面,获取了第一手资料,调研结果显示:农民工医疗保障的需求和供给严重失衡;吉林省现有的医疗保障政策落实不到位;资金不足将成为一大隐患;社会认识上对农民工的排斥也阻碍了农民工医疗保障的发展进程。本文的框架主要分为五部分。第一部分介绍了与农民医疗保障相关的理论和国内学者们关于农民工及农民工医疗保障的各种观点。第二部分介绍了农民工的概念及产生、农民工的主要特征及他们对医疗保障的特殊要求、并介绍了国内几个典型城市的农民工医疗保障的基本模式。第三部分提出了吉林省农民工医疗保障的现状并分析了建立吉林省农民工医疗保障的障碍。第四部分提出了建立吉林省农民工医疗保障的基本原则,包括立足于满足最基本需要的原则、权利和义务相统一的原则等,并提出了完善吉林省农民工医疗保障的具体对策。第五部分为结论,认为应根据农民工的特点和实际要求,从吉林省的实际情况出发,循序渐进,实施切实可行的农民工医疗保障措施,逐步完善吉林省社会保障体系。本论文的研究主要是针对吉林省农民工医疗保障所出现的问题进行的,通过研究以期达到解决现有问题、提高吉林省农民工医疗保障水平、完善吉林省社会保障体系的目的。

【英文摘要】 The migrant workers group has emerged as a special group after China's reform and opening up.Migrant workers are to refer to these workers who have rural area registered permanent residence dignity but are engaged in working in the cities and towns.Because traditional dual urban-rural economic structure and the household registration system,their dignity does not get broad accept ion.They often do not enjoy the same treatment as the urban people.Many institutional obstacles often restrict their survival and development.One of the most typical phenomenons is that migrant workers who need of medical protection urgently are excluded from society medical security.Migrant workers problem has aroused society attention gradually and there are more and more callings on the community to care for and help migrant workers.From different angles,domestic researchers have carried out various analyses on China's migrant workers problem.By contrast horizontal analysis,this article used social theory,group theory,system and order theory and conflict theory,and combined the above theories with specific problems of social health protection.Basing on the existing situation of Jilin province,the article carried out exploration seriously and discovered obstacles that are affecting the migrant workers' medical security in Jilin province.Finally,by the fact of inside and outside factors including Jilin province pertinent policies and regulations,society subjective impression,migrant workers' oneself characteristic and etc,made the solution of Jilin Province migrant workers medical problems of the basic principles and specific measures:(1) eliminating the impact of urban and rural household registration(2) improving the relevant laws and regulations(3) making the medical insurance fund of Jilin Province to preserve and increase the value-added(4) doing the statistical work of migrant workers in Jilin Province and the research work and a number of issues on policies and proposals.In the course of the research,we have obtained materials from 100 migrant workers,50 members of the public in Changchun and Tonghua,and 20 typical working units and 10 communities in Jilin Province, concerning age,physical health status,education levels,security needs,employment,health insurance status. The research results show that serious imbalance between demand and supply of migrant workers medical protection;the affection of implementation of Jilin existing medical protection polices is not well;inadequate funding will be a big hidden problem;social awareness on the exclusion of migrant workers have also hindered the development of the health care process.In this paper,the framework is divided into five main parts.The first part introduced the theories of migrant workers medical protection and the viewpoints of domestic scholars on migrant workers and migrant workers medical protection.PartⅡintroduced the concept of migrant workers and production,the main features of migrant workers and their specific requirements of medical protection,and introduced the migrant workers basic medical insurance model in several typical cities.The third part introduced the status quo of the migrant workers medical protection of Jilin Province and obstacles of developing.PartⅣhas proposed basic principles of the establishment of medical protection for migrant workers in Jilin Province,including the principle of based on the need to meet the most basic needs,the principle of the unity of rights and obligations, and made specific countermeasures to perfect migrant workers medical protection in Jilin Province.PartⅤmade the conclusion that it should be based on the characteristics of migrant workers and the actual requirements and the actual conditions in Jilin Province the to implement practical measures to protect migrant workers,and gradually perfect the social security system in Jilin Province.In this paper,the research is mainly directed against problems of migrant workers medical protection in Jilin province.The aim of the research is to resolve the existing problems,and enhance the level the migrant workers medical security in Jilin Province,and improve the social security system in Jilin Province.
